Forms collect information: an exit interview, an equipment request, an update to personal details. Services turn a form into a request employees can make, with an owner, a deadline and optional approval: for example Request a payslip letter or Report an IT issue. Both are under More.
Build a form
Your forms, with their fields, status and responses
- Open More → Forms and select Create Form.
- Start from scratch, or select Templates for a ready-made form: employee information update, onboarding questionnaire or exit interview.
- Add questions from the panel on the left: text, number, email, phone, link, dropdowns, checkboxes, yes/no, ratings and scales, dates and times, file uploads, signatures, and layout items such as sections.
- Select a question to set its title, whether it's required, its limits, and When to show it, to show it only when earlier answers match.
- In the form's settings, set the submit button text and colour, and what happens after submission: a confirmation message or a link.
- Select Preview to try it, then Finish.
Changes are saved as you go. Use undo and redo freely.
Share a form and see responses
- Select Publish to start accepting responses, then Copy link to share it. Close form stops responses.
- Open a form and switch to Responses to see who submitted and when. Open a response to read it, or select Export CSV.
View Archived shows archived forms. Archive keeps their responses; Delete Permanently removes everything.
Offer a service
- Open More → Services and select New Service.
- Name and describe it, add an image or icon, and choose a Category and tags so employees can find it.
- Request handling:
- the Request Form employees fill in (build it first, in Forms);
- the Service Owner and Assignment Group: who handles requests;
- the SLA (hours): how quickly requests should be handled;
- the Priority.
- Access & instructions: make it Public (anyone can request it) or Restricted to chosen roles, and add instructions for requesters.
- Approval workflow: add approvers in order, or leave it empty if no approval is needed.
- Select Create Service.
Group services with Categories, on the Categories tab.
Handle service requests
Open More → Services and select View requests. Filter by status or date, or search by service or email.
- Open a request to see the submitter and their form responses.
- Select Update status, choose the new status, add a note if useful, and confirm: Approve request, Reject request, Mark as completed and so on.
